Відїовідш до умов дашго додатка ор^ сертифікації їoви^e^ видати сертифікат відїовід- тості (сертифікат відїовідшсті ЄС), який да° їраво виробмку ^а^oсити СЕ-маркувашя. Цей сертифікат їoви^е^ вкёючати ^астуї^е: - ^азву, адресу і код ор^у сертифікації;
^азву і адресу виробмка або його уїовш- важешго в ЄЕЗ (Євроїейська екo^oмiч^а зo^а) їредставмка і місце виробмцтва;
оїис виробу (тиї, їoз^аче^^я, використаж ^я тощо)
їоложєшя, яким відїовіда° виріб фаїрик- ёад, додаток ZA дашго ставдарту);
сїеціальж їоложєшя, що діють дёя вико- ристашя виробу (наприклад, їоложєшя їро використашя за їєвмх умов і тощо);
шмер сертифіката;
умови і строк дії сертифіката, якщо можёи- во;
їрізвище і їосадах особи, яка ма° їраво їідїису сертифіката.
Окрім того, виробмком ^ада°ться декёарація відїовідшсті (декёарація відїовідшсті ЄС) ^астуї^oгo змісту:
^азва і адреса виробмка або його уїовш- важешго у ЄЕЗ їредставмка;
^азва і адреса ор^у сертифікації;
оїис виробу (тиї, iде^тифiкацiя, застосу- вашя,...) і коїія ^формації щодо СЕ-марку- вашя;
їоложєшя, яким відїовідають вироби ^а- їриклад, додаток ZA дашго ставдарту);
сїеціальж їоложєшя, що діють дёя засто- сувашя виробу (наприклад, їоложєшя їро застосувашя за їєвмх умов і т.д.)
шмер додашго (їрикладешго) сертифіката відїовідшсті ЄС
їрізвище і їосада особи, яка ма° їраво їідїису декларації від імені виробмка або його уїовшважешго їредставмка.

ZA.2.3 EC Certificate and declaration of conformity
(In the case of products under system 1)
When compliance with the conditions of this Annex is achieved, the certification body shall draw up a certificate of conformity (EC Certificate of conformity), which entitles the manufacturer to affix the CE marking. The certificate shall include: - name, address and identification number of the certification body
name and address of the manufacturer, or his authorised representative established in the EEA, and place of production
description of the product (type, identification, use)
provisions to which the product conforms (e.g. Annex ZA of this Standard)
particular conditions applicable to the use of the product (e.g. provisions for use under certain conditions etc.)
the number of the certificate
conditions and period of validity of the certificate, where applicable;
name of, and position held by, the person empowered to sign the certificate
In addition the manufacturer shall draw up a declaration of conformity (EC declaration of conformity) including the following:
name and address of the manufacturer, or his authorised representative established in the EEA
name and address of the certification body
description of the product (type, identification, use, ), and a copy of the information
accompanying the CE marking
provisions to which the product conforms (e.g. Annex ZA of this EN)
particular conditions applicable to the use of the product (e.g. provisions for use under certain conditions etc.)
number of the accompanying EC Certificate of conformity
name of, and position held by, the person empowered to sign the declaration on behalf of the manufacturer or of his authorised representative.
(In the case of products under system 3)
When compliance with the conditions of this Annex is achieved, the manufacturer or his agent їoви^e^ оформити і зберігати декёарацію відїовідшсті (декёарацію відїовідшсті ЄС), котра да° їраво виробмку ^а^оcити СЕ-мар- кувашя. Ця декёарація вкёюча° в себе ^а- стуше:
^азву і адресу виробмка або його уїовш- важешго у ЄЕЗ їредставмка і місце ви- робмцтва;
оїис виробу (тиї, їоз^аче^^я, використаж ^я і тощо) і коїію суїровідшї ^формації до СЕ-маркувашя;
їоложешя, якому відїовіда° виріб фаїрик- ёад, додаток ZA дашго стандарту);
сїеціальж їоложешя, що діють дёя засто- сувашя виробу (наприклад, їоложешя їро використашя за їєвмх умов тощо);
шмер і адресу усташви (усташв), яка ви- даёа дозвіё;
їрізвище і їосаду особи, яка ма° їраво їідїису декларації від імені виробмка або його уїовшважешго їредставмка.
^аведе^а вище декларація і ^аведе^ий вище сертифікат їовиші оформлюватись офіційою мові (мовах) держави-участика, в якій виріб за- стосову°ться.
ZA.3 СЕ-маркувамя і табёички/етикетки
За ^а^еcе^^я СЕ-маркувашя відїовіда° ви- робмк або його уїовшваже^й в ЄЕЗ їред- ставмк. СЕ-маркувашя у сво°му символічшму зображені їови^е^ відїовідати директиві 93/68/ЄС, і їови^е^ вказуватись у суїровідмх докуме^ах, ^аїриклад, у ^аклад^ій. До СЕ-маркувашя їовиша докладатись ^аcтуї- ^а ^формація:
код ор^у сертифікації (тільки для виробів відїовідш до системи 1);
^азва або з^ак і заре°cтрова^а адреса ви- робмка;
осташя цифра року, в якому з^ак було ^а- ^еcе^о;
шмер сертифіката відїовідшсті ЄС або сертифіката FPC (якщо ^еобxід^о);
їосилашя ^а дамй ставдарт;
оїис виробу: їов^а ^азва, матеріал, розміри, ... і використашя за їриз^аче^^ям
^формація їро важливі вагомі властивості у таблиці ZA.1;
established in the EEA shall prepare and retain a Declaration of conformity (EC Declaration of conformity), which entitles the manufacturer to affix the CE marking. This declaration shall include:
name and address of the manufacturer, or his authorised representative established in the EEA, and place of production
description of the product (type, identification, use), and a copy of the information accompanying the CE marking
provisions to which the product conforms (e.g. Annex ZA of this Standard)
particular conditions applicable to the use of the product (e.g. provisions for use under certain conditions etc.)
name and address of the approved labora- tory(ies)
name of, and position held by, the person empowered to sign the declaration on behalf of the manufacturer or of his authorised representative.
The above mentioned declaration and certificate shall be presented in the official language or languages of the Member State in which the product is to be used.
ZA.3 CE marking and labelling
The manufacturer or his authorised representative established within the EEA is responsible for the affixing of the CE marking. The CE marking symbol to affix shall be in accordance with Directive 93/68/EC and shall be shown on accompanying documents, e.g. a delivery note. The following information shall accompany the CE marking symbol:
identification number of the certification body (only for products under system 1)
name or identifying mark and registered address of the producer
the last two digits of the year in which the marking is affixed
number of the EC Certificate of conformity or factory production control certificate (if relevant)
reference to this European Standard
description of the product: generic name, material, dimensions,.and intended use
information on the relevant essential characteristics in table ZA.1

Вказівку "Робоча характеристика ^е виз^ача- °ться" (РX^В) ^е мож^а використовувати, якщо характеристика ма° граничні з^аче^^я. В Н0их виїадках мож^а використовувати вказівку РX^В, якщо характеристики дёя пев- мго з^аче^^я ^е підёягають закомдавчому регёаме^увашю.
Додатково до ^аведе^ої вище спеціаёьмі' ^формації їро ^ебезпеч^і вибухові речовим, за ^еобхід^ості, повима бути дода^а у прий- ^ят^ій формі докуме^ація, в якій ^аводяться всі ^0Ї акти їро ^ебезпеч^і речовим, а також ^формація, яка вимага°ться цими закомдав- чими актами. їримітка: Євроїейські заком- давчі акти, що ^е відріз^яються від ^аціо- ^аёь^их, ^е їотрібм вказувати.

values, and, where relevant, level or class to declare for each essential characteristic as indicated in table ZA.1
as an alternative, values presented as standard designations
characteristics against which the "No performance declared" (NPD) option is relevant.
The "No performance declared" (NPD) option may not be used where the characteristic is subject to a threshold level. Otherwise, the NPD option may be used when and where the characteristic, for a given intended use, is not subject to regulatory requirements.
In addition to any specific information relating to dangerous substances shown above, the product should also be accompanied, when and where required and in the appropriate form, by documentation listing any other legislation on dangerous substances for which compliance is claimed, together with any information required by that legislation. Note: European legislation without national derogations need'not be mentioned.
Figure ZA.1 gives an example of the information to be given on the accompanying commercial documents
